Dear students,
Most of you have already completed summer student internship. Completing the internship and entering the grade into the USOS requires submitting documents to the appropriate internship guardian. Dr Wojciech Ulrych is the person to whom you are to provide the documentation. The deadline for submitting scans is January 15; however, do not leave it until the last minute, which may be risky if your documents are incomplete.
Below you can find the instructions that include four stages that we ask you to follow so that the submission process goes smoothly.

STAGE 1


Please complete a set of documents completed in accordance with your chosen internship option. The list of required forms and their corresponding options can be found here (internship options) and here (internship regulations)

 

Note 1!
If you did your internship prior to or later than the formal period (i.e. July–December), it is necessary to add to the documentation this Attachment 6. It applies to completing the internship programme started at an earlier date / or the internship programme completed before the formal period. 
 

Note 2!
Your internship supervisor will decide how you can provide the internship documentation. It could be in person, or by MSTeams or Moodle.

STAGE 2

Please scan the documents (each separately) in a pdf file and name the file as follows:
Student ID number,_surname_name,_field of study

STAGE 3

The scans must be uploaded or sent in accordance with the instructions of your supervisor (e.g. MSTeams, Moodle, e-mail). The order of attached scans in the file should be in accordance with the forms listed in your chosen option. It significantly facilitates the work of the supervisor as well as is a way for you to self-check whether all documents have been attached (remember to add attachment 6 if it applies to you).

STAGE 4

After you have received information from the supervisor about the correctness and completion of the documents (you will receive 1 point for the completed task), their originals must be placed in an A4 envelope and sent by registered mail to the address of the Faculty of Management of the University of Lodz, or should be delivered to the parcel box with the name of the guardian on it. The parcel box is located in the Faculty building at concierge lodge on the ground floor. 
The envelopes must be addressed according to the pattern below:
